Dear Colleague
I welcome endodontic referrals for consultation and/or treatment. Following your referral, I will write to your patient with details about the consultation and arrange for a mutually convenient appointment. At the consultation a comprehensive history, examination and assessment will be carried out and a diagnosis made. I will discuss the various treatment options and provide the patient with information so that he/she can make an informed decision.
Subsequently the discussion will be relayed to the patient by way of a letter and I will be sending you a report as well. After completion of treatment, I will discharge the patient to you with a completion report advising of any necessary adjunctive treatment and after care.
Any referrals can be made either by email or fax/post. You can either print the referral form (pdf) for fax/post or use the form (Word) for email. Any relevant radiographs or photographs can be sent electronically or by post and these will be returned after completion of treatment.
