Ultrasonic instrumentation has been in use in dentistry for a while. The use of ultrasonics in endodontics has been more recent. The ultrasonic unit attaches to small diamond coated tips which vibrate at around 20,000 cycles per second. The small size of the tip allows more visibility of the tooth particularly when the microscope is used. Ultrasonic is also useful in biomechanical preparation of the root canal. It causes 'cavitation' and 'accoustic microstreaming' within the irrigant in the root canal which enhances the cleaning effect of the irrigant.

The removal of tooth structure and other materials is gradual and controlled while using the ultrasonic. They can be used in locating small canals and gently removing objects from previous therapy including posts and filling materials, cleaning and shaping and in obturation of root canals.

The ultrasonic equipment can also be used in endodontic surgery along with magnification to obtain a predictable result. Specialised ultrasonic tips are used in conjunction with micro-surgical instruments and mirrors which help in visualisation of the surgical area using the microscope.
